The equation


Ax+By+C=0

is said to be the general form for the equation of a line.

Then in this case the general form is:


4x+4y+5=0

A line is said to be in point slope form if it is written as:


y-y_1=m(x-x_1)

In this case we have:


\begin{gathered} 4x+4y=-5 \\ x+y=-(5)/(4) \\ y=-x-(5)/(4) \end{gathered}

Therefore the equation is point slope form is:


y=-x-(5)/(4)
