Explanation:

you are already partially there with finding the slope!

use the formula


(rise)/(run)

rise means the height you are climbing upwards

run means the distance you are traveling sideways

you already determine that your rise = 2, and your run = 4.


slope=(2)/(4) =(1)/(2)

to complete the slope-intercept form, you need the y-intercept. to find it, you set x=0


y-intercept=-5

(we can see this based on the graph)

with all the components you can make your slope-intercept form!
