at the end (!) he had 4 left.

this does NOT mean he lost 4 balls, it means that he lost some balls (x) had at the end 4 balls remaining.

so, it is

12 - x = 4

12 = 4 + x

x = 12 - 4 = 8

he lost 8 balls.
