Why can the efficiency of a machine never be 100 percent?

A. Energy isn't conserved.
B. The power used can't be measured accurately.
C. Machines can't transform energy from one type to another.
D. Friction converts some energy into a useless form.

The answer is D because in a machine, there is always some loss of energy due to friction between different parts of the machine, as a result of which the output of the machine is always less than the input. Hence, no machine is 100% efficient.
