y = (3/4)x + 5

Using the two coordinates, you can find slope and y-intercept

y-intercept is 5, from the point (0,5)

slope is 3/4, from rise/run, and that 0 to 4 is 4, while 5 to 8 is 3.

Combine slope and y-intercept for slope intercept form

y = (3/4)x + 5
