After you ask someone if they are okay and they say " Yes, just got a lot going on " Can you reply back " What's going on? " or that's disrespectful?

It is not disrespectful, if anything it is showing your care and concern for them. Depending on the tone of your voice when you say it could determine if it really is disrespectful or not.
