The point-slope form would be:

slope-intercept form: y=-3/4x-9

point slope form: y-6=-3/4(x+4)

You had -3/4x in your original equation, but then put it as m=3/4 on the bottom. Depending on which one it is that is what you put in for 3/4(x+4)
