I would believe in urban.
As this is a town and city, bigger place.

Disease is easier to spread in an urban society because people are closer together and germs can be contracted easier. Think of the bubonic plague. The people that were hit the hardest were those that lived in more developed areas because everyone is in such close contact with each other
