The slope-point form of the line equation is

y-b=m(x-a)

If we replace the slope m=-1/2 and the point (4,-5), that is, a=4, b=-5, then we obtain

\begin{gathered} y-(-5)=-(1)/(2)(x-4) \\ y+5=-(1)/(2)x+2 \\ y=-(1)/(2)x-3 \end{gathered}

This is the slope-intercept form of the equation, we can also write it in the next way

\begin{gathered} y=-(1)/(2)x-3 \\ 2y=-x-6 \\ x+2y=-6 \end{gathered}
