The point-slope form:

y-y_1=m(x-x_1)
m - the slope, (x₁,y₁) - the coordinates of a point


m=-5 \\ x_1=-2 \\ y_1=4 \\ \\ y-4=-5(x-(-2)) \\ \boxed{(y-4)=-5(x+2)} \\ \\ y-4=-5(x+2) \\ y-4=-5x-10 \\ 5x+y=-10+4 \\ \boxed{5x+y=-6}

The answer is B and C.
