Step 1

State the point-slope formula

y-y_1=(x_2-x_1)/(y_2-y_1)(x-x_1)

Step 2

\begin{gathered} y_1=-4 \\ y_2=1 \\ x_1=5 \\ x_2=0 \end{gathered}

Step 3

Substitute and get the equation of a line

\begin{gathered} y-(-4)=(1-(-4))/(0-5)(x-5_{}) \\ y+4=(5)/(-5)(x-5) \\ y+4=-1(x-5) \\ y=-x+5-4 \\ y=-x+1 \end{gathered}

The required equation is y=-x+1
