To create an equation, you have to find the slope of the line

m =
( y_(2)- y_(1) )/( x_(2)- x_(1) )

Where m is your slope and:

(4,5) =>
(x_(1), y_(1))

(-3,-1) =>
(x_(2) , y_(2) )

So, plug the numbers into the equation

m = (-1-5)/(-3-4) = (-6)/(-7) = (6)/(7)

The slope of this equation is 6/7

Point slope form of a line is:

y - y_(1) = m(x - x_(1) )

(-3,-1) =>
(x_(2) , y_(2) ) and m = 6/7

y + 1 = 6/7 (x + 3)
