What kind of sentence structure is this: Once upon a time, there was a town where all the citizens were fruits and vegetables.

Complex Sentence. A complex sentence has at least one independent clause plus at least one dependent clause. "Once upon a time" is the dependent clause and "there was a town where all the citizens were fruits and vegetables" is the independent clause.
