According to the given data we have the following:

equation of the line that passes through the point (-8, -12)

slope m=6

equation of the line is y=mx +b

Therefore, to find the equation in point-slope form and slope-intercept form would we would plug the point (-8, -12) and the slope 6 at the equation above, so:

-12=6(-8) + b

-12=-48 +b

b=-36

Therefore, y=6x -36
