b) has been to Italy
This should be the answer because it has to be in past tense. "has came" is also is past tense, however the grammar is incorrect.

Answer: A. has gone

The verb phrase " has gone" is the correct phrase to be supplied in the sentence to make it grammatically correct. Therefore, the complete sentence is this, "Tom is just back from holiday; he has gone to Italy."
