The point-slope form is

y-y_1=m(x-x_1)_{}

where m is the slope and (x1,y1) is a point where the line passes through

In our case

m=5

(x1,y1)=(-9,2)

Then we substitute

y-2=5(x+9)

ANSWER

The point-slope form is

y-2=5(x+9)
