From what I learned, I can say if plaque is not adequately removed from the surface of the tooth, it can cause:
Tooth decay
periodontal diseases ( bacterial infection of the gums)
Halitosis or bad breath

Answer:

When plaque is not remove from teeth it causes tartar, which is a hard yellowish that adheres to the tooth. The surface on the tooth is so hard that only can be removed by a professional. After tartar appears, if it's not removed, it can lead to gum diseases as gingivitis or cavities, if this isn't treated at time the person can lose the tooth.
