What two factors are usually the most responsible for determining population growth?

Population growth is usually caused by 1) the increase in the number of births/birth rate and 2) the decrease in the number of deaths, usually caused by an improvement of health care or a peace period after a period of war.

Additionally, population growth in a given area can be caused by immigration into this area.
