If helen mistakenly accepts her null hypothesis when it should be rejected, what has occurred?

This is a Type II error. In this case, a "false negative" has occurred. The null hypothesis has been shown to be accepted (or, no effect has been shown) when there should have been some statistically significant effect. This is likely an error in the creation of the experiment.
