(a.) since x >= 2002 , x needs to be the year not the number of years after 2002.
y = -33(x-2002) + 3039
y = -33x + 66066 + 3039
y = -33x + 69105
(b.) Use x = 2012 in the equation
y = -33(2012) + 69105
y = -66396 + 69105
y = 2709 hospitals
