What’s the point slope of (0,2); m=4

The point slope formula is:
y - y₁ = m(x - x₁)

m = 4
(x₁, y₁) = (0, 2)

y - 2 = 4(x - 0)
If they only want the point slope form then this is the answer.

If they want the slope intercept, all you do is simplify.
y - 2 = 4x
y = 4x + 2
