we have

x^2+ 12x + 4 = 0

so

(x^2+12x+6^2-6^2)+4=0

Rewrite as perfect squares

(x+6)^2=36-4

(x+6)^2=32

take the square root both sides

\begin{gathered} x+6=\pm\sqrt[\square]{32} \\ x=-6\pm\sqrt[\square]{32} \end{gathered}

Simplify

\sqrt[]{32}=4\sqrt[\square]{2}

therefore

the solutions are

undefined
