I believe the answer is D-Neonatal deaths account for a smaller share of under-5 mortality.

In consideration that a country had an increased health awareness and practices, among all the choices letter d. shows one possibility. Neonatal deaths or newborn deaths may decrease and would become a small percentage of under 5 mortality rate.
