it might be said that his melancholic tone highlights the despair of the excerpt (option b). This narrator seems a bit morbid in a way, it might be connected with a gloomy ans sad atmosphere. And the choice of words seems to contribute to this tone, for example "fingers were deeply imprinted the flesh" , "dying agony".
