We have that the point-slope form is


y-y_1=m(x-x_1)

where (x1,y1) is a point that the line pass through and m is the slope

In our case we have

(4,6)=(x1,y1)

m=5

we substitute the values and we obtain the point-slope form


y-6=5(x-4)

then we need to isolate the y in order to have the slope-intercept form


\begin{gathered} y-6=5x-20 \\ y=5x-20+6 \\ y=5x-14 \end{gathered}

the slope-intercept form is y=5x-14
