By asking caregivers about self-care items taken from the dpiyc in a question and answer format are you using this tool as a standardized test?

No. A standardized test  (1) requires test takers to answer similar questions, in the same way, and that (2) is scored in a standard or consistent manner. It is usually given in multiple-choice format, true-false questions, short-answer questions, or essay questions but not citing examples like self-care items,  which cannot provide a standard assessment to caregivers.
