slope m = 5
point (5,6) lies on the graph of h(x)
so
y = mx + b
6 = 5 (5) + b
b = 6 -25
b = -19

equation
h(x) = 5x - 19

h(6) = 5(6) - 19 = 30 - 19 = 11

answer
h(6) = 11
