A client who had an esophageal hernia repair 4 hours ago has a pulse rate of 90 bpm, respiration rate of 16 breaths/min, blood pressure of 130/80 mm hg, pulse oximeter of 91%, and a temperature of 100.4° f (38° c). what should the nurse do first?

It looks like most of the vital signs are fairly normal except for a slightly high heart rate so the nurse would probably try to bring that down say by rest or medication being mindful that the body is still stressed after a recent operation so needs extra care (after serious operations patients often have even intensive care but this would probably more for patients with more serious operations).
