The nurse should give the patient two tablet of misoprostol each time.

Since a tablet consist of 100mcg misoprostol, 200mcg would be 200mcg/100mcg= 2 tablet.
Taken 4 times a day mean that the patient should take the drug every: 24hours/4= 6 hours.
