If they give you a point and slope, use point slope form.
Then simplify to get slope intercept form (y = mx + b)
slope (m) = 5
(-2, 1)

y - y1 = m(x - x1)
y - 1 = 5(x + 2)
y - 1 = 5x + 10

y = 5x + 11
