The nurse should take the log roll step from side to side during the immediate postoperative period. Herniated nucleus pulposus or herniated disc is a soft material that is displaced into the spinal canal. The symptoms include pain in the lower back portion of the body, muscle weakness, numbness, pins and needles sensation and loss of reflexes. The best way to fight this is to physical therapy and exercise.
