1. In my opinion this statement is false. Information entered into the electronic health record such as patient demographics can be used for various purposes such as billing information or for the filing of insurance claims. It helps to get rid of extra paper work.

2. I guess this one is also "true/false" question. This statement is true. Depending on the kind of disease information from electronic health records can be used at the local level, at the state level, and at the federal level. For example communicable diseases need to be reported at the local level.

3. As far as I remember, complete statement should look like this:
