A patient with a history of a t2 spinal cord tells the nurse, "i feel awful today. my head is throbbing, and i feel sick to my stomach." the skin is flushed and diaphoretic, and the patient is anxious and restless. which action should the nurse take first?

I had to look for the options and here is my answer:
Based on the given history and chief complaints of the patient, I can say that the first thing that the nurse should do is to CHECK THE BLOOD PRESSURE. Since the damage is at the T2 level, patients complaining about a headache should be assessed for they might be experiencing autonomic dysreflexia and this would result to an increased in BP.
