A. While I was brushing my teeth, a spider crawled on the sink. You can tell because of the perspective/point-of-view in the subject. B, C, and D all make it seem like the spider is brushing your teeth while crawling into the sink. Even though C is very similar to A, the "I was" can make a huge difference in your sentence's composition.
