These roadblocks are usually caused by organizational structure failures, may be due to employee insecurity in accepting changes, this may be caused by training failure or lack of motivational incentives to help the employee understand changes. Resistance to change often has a previous history of unsuccessful change, lack of clarification and feedback.

When you say "we've never done it that way before", this may reflect your thoughts. It's either you don't want to consider another way to do things or you are willing to entertain the idea of change. This is one of the internal roadblocks to effective listening, when you think that you don't want to hear what the speaker has to say, you will lose interest in listening to him.
