The slope is -3. So you just plug in what you know which is the (x1,y1). So the point slope form is y+1=-3(x-4).
The slope intercept form is y=mx+b. So you plug in the m,x, and y to solve for b. B is 11. So the slope intercept form is y=-3x+11.
