for ax²+bx+c=0
x=
(-b+/- √(b^2-4ac) )/(2a)

so

16x²+8x+1=0
a=16
b=8
c=1

x=
(-(8)+/- √((8)^2-4(16)(1)) )/(2(16))
x=
(-8+/- √(64-64) )/(32)
x=
(-8+/- √(0) )/(32)
x=
(-8+/- 0)/(32)
x=
(-8)/(32)

x=-1/4
