Ok, you are going to click File, options, customize ribbon, select the list of main tabs, then select the developer tab and make sure it has a check mark next to it.
Now that you have done that you need to click file, new, and search for the template you want to use in the search template box, then click create. Now you need to click file, new, blank document, at the top of your screen under developer click design mode, then you need to click on the page where you want your form to be, then click on the developer tab and choose one of the text controls.
