Read the passage.

He saw all spoiled. “Don’t let him cut my hand off—
The doctor when he comes. Don’t let him sister!”
So. But the hand was gone already.
Why does Robert Frost deviate from iambic pentameter in this section of “Out, Out—”?

He wants to make this section of the poem include rhyme.
Deviations are required when a character speaks in a poem.
These deviations are essential elements of blank verse.
He wants to emphasize the gravity of the boy’s situation.

After reading the poem, it seem so me that the reason why Robert Frost deviates from iambic pentameter in this section of “Out, Out—” is being shown in the last option from the scale represented above :He wants to emphasize the gravity of the boy’s situation. As you can see, there is a tense situation in the centre of composition, so author need to create sort of suspense and make reader feel this pain.
