Fill in the term column first. As you can see 3 of the terms are filled in, so you should be able to figure out what the 2 other terms are.
A coefficient is the number in front of a variable. So the second column wants you to look at the term in the first and write the number you see in front of the variable.
The degree is the exponent of your term.
The degree of the polynomial is the highest exponent of the polynomial.
(A constant, plain number with no variable, does not have a coefficient and is degree 0)
