Option 1: A pre-printed document that contains spaces in which the user enters information.

A workplace form can be defined as a printed document with blank spaces for information to be inserted, related to the work area. They are usually delivered from an employer to an employee. There are a variety of workplace forms since they are structured depending on the information that needs to be recollected. Some examples are forms prior to seeing the Doctor for Work Cover certificate, forms to complete general medical history, a form to collect basic data such as Address, phone number, relatives, etc.
