The following are some of the main reasons that patients have a poor outcome or at higher risk for certain diseases and suggested solutions.

- Genetic factors: Can’t be modulated

- Less access to health care providers because of far distance for example: First identify these neighborhoods and send mobile clinic that meets their need on regular bases.

- Greater exposure to environmental toxins: Raise the awareness of the citizens of the commonly known toxins in their area in commercial ads in TV, newspaper, and radio. There should be a routine inspections form the department of environmental health (DEH) in neighborhoods that are known to rodents/bugs infestation. This will offer help to the citizens and increase their awareness.

- Unhealthy lifestyle: Commercial ads will encourage healthy eating. Make healthy meal options readily available at lower cost.

For example, the percentage of kidney dialysis in the United States is considerably higher in poor areas. According to the American Cancer Society, the rate of colon cancer is much higher in the following regions compared to the rest of the country; North Carolina, west-central Appalachia and in the Mississippi delta.
