Heparin acts as a cofactor for antithrombin. Antithrombin inhibits the formation of thrombin, which in turn inhibits the conversion of fibrinogen to fibrin. When fibrin is absent, platelet plugs which are unstable will eventually come apart, hence blood clotting does not occur.
