1:4 is the answer.
The common factor of 3 and 12 is 3.
3 divided by 3 is 1
12 divided by 3 is 4
So you get 1:4

The ratio 3:12 in simplest form would be 1:4. We can figure this out by dividing the ratio by 3 which would leave 1 plate to 4 napkins.
