Question marks are used at the end of interrogative sentences. Interrogative sentences pose a question to the reader and the speaker is looking for information.

Sometimes when asked questions like the one that you are, it can help to put a question mark at the end of each sentence choice:
A. (9) Sadly, even after that water is found, only some of its clean and safe enough to drink?
B. (10) A number of groups across the globe have spent decades helping people get better access to water?
C. (1) Water is something most of us take for granted?
D. (13) What have they accomplished so far?

You may already see which one is the best choice! You can also ask yourself, "Which sentence is asking me a question? Which one shows that the speaker is looking for more information?" Adding a question mark to choice D would make the most sense. Choice D is an example of an interrogative sentence. It is asking a question to you as the reader and shows that the speaker is looking for more information.
