The last one has to be correct since is has the word “had” in it (has is the spanish word for had in this case). If you said “I had a dog”, it would be “tenía” not “has”

D- Te has puesto una inyeccion? (Sorry I can't do accents on my computer but the o in inyeccion has an accent)
