Oni works at a restaurant. They earn 12.01 per hour plus a 4.05 each time someone signs up for the store's credit card. Write a formula to help Oni determine what they will make in one week. Let: a = total amount earned. h = hours worked. c = number of cards sold in that week.

I think you take 12.01 and multiply by "h" the hours, and then 4.05 by "x" for the people signing up. So I think it is 12.01h+ 4.05a. Does that sound right? You probably know this better than me ;)
