The point-slope form:

y-y_1=m(x-x_1)

We have m = -4 and the point (-2, 5). Substitute:

y-5=-4(x-(-2))

y-5=-4(x+2) use distributive property

y-5=(-4)(x)+(-4)(2)

y-5=-4x-8 add 5 to both sides

\boxed{y=-4x-3}
