It would be 40 because 1+4 equals 5. On the next question, add 5+2 and your answer would be 7. They got twelve by adding 5+7. So, take the 21, and add 8+11. 8+11 equals 19, so add 21 to it and it would equal 40. Hope this makes sense! :)
