y-y_1=m(x-x_1)

We have the point (-2, 5) and the slope m = -4. Substitute:


y-5=-4(x-(-2))


y-5=-4(x+2) use distributive property


y-5=-4x-8 add 5 to both sides


y=-4x-3 add 4x to both sides


4x+y=-3

Answer:

point-slope form: y - 5 = -4(x + 2)

slope-intercept form: y = -4x - 3

standard form: 4x + y = -3
